PEIA PATHWAYS TO WELLNESS LIFESTYLE CHANGE PROGRAMS

The PEIA Pathways to Wellness Program encourages participating worksites to take advantage of our lifestyle change programs (formally known as behavior change programs). Worksite coordinators, on behalf of their employees, may choose a maximum of one program per health screen year that incorporates a tool. Worksites may also participate in the programs that do not incorporate a tool. Eligible employees are those who have participated in a health screen within the last year (this may include one or all of the following: blood pressure, osteoporosis screening or blood work).

Please note that this represents a change in the way lifestyle change services are delivered due to the new Improve Your Score program where participants may earn a cash rebate card if they score in the green (healthy) or yellow (moderate risk) range based upon an aggregate score for cholesterol, blood pressure, Body Mass Index and glucose. Receipt of the cash card now will serve as an incentive for improving your health!

Fitness Program with Tools:
  1. Stepping Stones – Over a six week period, participants will use a pedometer to count and record their daily number of steps. The program can be individual or team based depending upon the site’s preference. Each participant must complete a pre and post survey and walking log.

  2. Dance Challenge – Using the very popular Dance Revolution or Dance Factory (you can download your own music) participants will be provided either the software or a dance mat for this four week program. Participants will also receive: an instructional DVD, a tracking log, and a pre and post attitudes towards exercise questionnaire.

  3. The Tone Zone – An eight week weight bearing exercise program where participants are provided a set of water weights and a laminated guide. They must complete a pre and post survey and keep a log.
Fitness Programs with No Tools
  1. Walk 100 Miles in 100 Days – This 14-week program is designed to help participants become more physically active by walking 100 miles in 100 Days. This program can be individual or team based depending on the worksite’s preference. Individuals must complete a pre and post survey and keep a log.

  2. Use It & Lose It: the Personal Trainer You’ve Been Looking For (WVPortions Online Course) – A 12 week exercise program using your body weight and free weights to become stronger and leaner. Dan Foley, an experienced sports and fitness trainer, walks you through a self-assessment to determine your level of fitness, then directs you to a 12 week workout plan that matches your own fitness level. You must complete an exercise log for your fitness level.
Nutrition Programs with Tools
  1. Dump the Junk – Utilizing the ever popular You on A Diet book by Dr. Oz, the Dump the Junk program requires participants to rid their diets of trans fats and high fructose corn syrup while emphasizing the consumption of fruits, vegetables and whole grains. Participants must complete a pre and post survey and keep a log.

  2. Chug A Jug – The message of this simple program is to increase water consumption while reducing the intake of soda and other sweetened drinks. It has proven to be one of our most popular programs with surprisingly good results in the area of weight loss. Participants must complete a pre and post survey and keep a log.
Nutrition Programs with No Tools
  1. Step By Step Plan to a Healthier You (WVPortions Online Course) – An easy to use question and answer program on the basics of nutrition. This convenient course will provide participants with a certificate upon completion.
Available through mini grants (one per every 3 years)
  1. CPR Friends and Family – A self-directed learning product that was developed by the American Heart Association to increase CPR knowledge and awareness among the general public – those who do not traditionally attend CPR courses. Currently:

    • Almost 80 percent of cardiac arrests occur at home and are witnessed by a family member.
    • Only 6.4 percent of sudden cardiac arrest victims survive because the vast majority of those witnessing the arrest are people who do not know how to perform CPR.

    CPR Anytime is a tool that can help increase survival rates. People who receive the CPR Anytime kit are able to share lifesaving skills by using the kit to train their family and friends.

  2. Work it Out exercise physiologist evaluations – Your worksite will have access to an exercise physiologist that will come on site to conduct personal fitness assessments. Participation in this program will require a doctor’s release. These assessments will take approximately 30 minutes per person and consists of: height, weight, blood pressure, muscular endurance test, flexibility test, strength test and aerobic conditioning. After completion, the exercise physiologist will prepare a personalized exercise program for each participant.

    These two new programs will be available on a ‘first come first serve’ basis. There is a standardized grant form that needs to be completed in its entirety before consideration for either program. All grant requirements including; completion of an annual health screen, successful completion of a lifestyle change program and submission of you annual operating plan still apply to this program.   3. Grants for other worksite specific health improvement activities
