Grant Proposal Guidelines

In January 2002, PEIA introduced a Grant Proposal Program to the PEIA Pathways to Wellness program. Grant proposals can be submitted to PEIA to request funds to offer a health promotion/behavior change program at the site or provide wellness services to its employees.

To be eligible to apply for a grant your worksite must have:

  • Participated in a health screen,
  • Submitted an operating/business plan, and
  • Successfully completed one lifestyle change program.
Your Health Promotion Consultant will be contacted to verify this information.

A Commitment to Wellness Pledge must be signed and returned with your grant application.

Each typed (handwritten grants will not be accepted) proposal must include:

  • A defined mission for the proposal/Title/Description (Be Specific - include speaker names and credentials, topic names, etc.)
  • A detailed budget (Be specific-include full item name, model number, and cost of item, including tax and shipping and handling if applicable.)
  • A rationale for why the proposal is being requested
  • Goals and objectives related to the proposal
  • Itemized list of products and services to be purchased (Be Specific - List company name(s), mailing address(es), telephone number(s) and which item(s) is/are being purchased.)
  • The number of participants that proposal will target
  • Evaluation tool (i.e. survey) to be used with the program - REQUIRED

Each item above should be explained more thoroughly in the actual grant proposal. A paper copy of the grant application may be obtained through your health promotion consultant or by downloading a printable version of the application form. You can also submit a grant proposal application by using the online grant application.

Before you fill out the grant application, it is recommended that you read these ideas for utilization of grants. Grants that contain items that are not recommended will be returned.

Grants will be awarded in amounts up to a maximum of $1,000.00. Larger grants on a case-by-case basis will be awarded to worksites with larger populations.

Proposals will be evaluated by a review committee on a quarterly basis.

Quarter 1: January – March
Deadline for submission: March 31
Reviewed in April and checks sent out

Quarter 2: April – June
Deadline for submission: June 30
Reviewed in July and checks sent out

Quarter 3: July – September
Deadline for submission: September 31
Reviewed in October and checks sent out

Quarter 4: October – December
Deadline for submission: December 31
Reviewed in January and checks sent out

One grant proposal per year is allowed. Those sites receiving grant funding are required to wait 3 years before reapplying.

Grant Funded Programs

Beginning May 2008, worksites will have the option to participate in the following programs that are funded through the Grant Program. As with the traditional grant, terms and conditions are subject to change without notice.

CPR Friends and Family

CPR Friends and Family is a self-directed learning product that was developed by the American Heart Association to increase CPR knowledge and awareness among the general public– those who do not traditionally attend CPR courses. Currently:

  • Almost 80 percent of cardiac arrests occur at home and are witnessed by a family member.
  • Only 6.4 percent of sudden cardiac arrest victims survive because the vast majority of those witnessing the arrest are people who do not know how to perform CPR.

CPR Anytime is a tool that can help increase survival rates. People who receive the CPR Anytime kit are able to share lifesaving skills by using the kit to train their family and friends.

WV Work It Out: Fitness Assessments

New research demonstrates that people who regularly engage in physical activity receive a number of benefits including:

- They are better able to achieve and maintain a healthy weight
- They have improved immune systems
- They are less prone to accidents
- They sleep better
- They have increased concentration when performing mental tasks
- They recover more quickly from injuries and infections
- They are better able to deal with stress

For all of these reasons, PEIA’s Work it Out Pilot Program has now changed from pilot status to an approved worksite grant option.

What Can Worksites Get Through Work it Out at Work?

Option 1

  • 30 Minute Fitness Assessment at Worksite
  • Exercise physiologist will prepare a personalized exercise program for each participant and a copy of Yourself Fitness (computerized personal training) or other tools will be provided as appropriate for the participant’s specific fitness level.

A personal trainer will assist the individual with the implementation of their fitness plan if the worksite has a fitness facility. Assessments provided on first come, first serve basis.

Option 2
  • 30 Minute Fitness Assessment at Worksite
  • Exercise class with a credentialed instructor will then be made available after the initial fitness assessments have been completed.
  • Classes will meet once weekly for 8 weeks at the worksite.
  • Limit of 15 people per worksite

How do we apply for Work it Out at Work?

Fill out a grant application indicating that you want to bring Work It Out at Work to your site. Please indicate whether your worksite has an onsite fitness center.

A paper copy of the grant application may be obtained through your health promotion consultant or by downloading a printable version of the application form. You can also submit the WV Work It Out grant application by using the online grant application.
