Saturday Morning Health Screenings

Thank you for your interest in desiring to participate in a PEIA Pathways to Wellness Saturday morning health screening at a participating West Virginia LabCorp facility. These screenings will occur the second Saturday of each month through June 2011. Each participating LabCorp location must meet an attendance requirement of 10 scheduled appointments 15 days prior to the scheduled screening date. Scheduled participants will receive an email notifying them of a cancellation if the attendance requirement is not met.

NOTICE: There is enough participation to warrant a Saturday morning health screening at ONLY the Huntington and South Charleston LabCorp locations on June 11th beginning at 8:00 a.m. The Saturday morning health screening option at participating West Virginia LabCorp facilities will be suspended until further notice due to declining participation. Please click on the Health Screen link to locate a wellness worksite near you.

Website Registration Instructions

By clicking the following link you will begin the registration process. You be directed to the registration portal where you will be asked to create an account. Simply clink the link to proceed with the registration process. www.bebetterhealth.net Once on this page, select Account Setup if you are registering for the first time. If you have already registered, simply enter your username and password and click login.

You will then be asked to enter basic information once you reach the Account Setup page. Please complete the first two fields accordingly.

Organization ID: 17 (Please use the number 17 in this field.)
Member ID: Please enter the entire number found on your PEIA insurance card. This number begins with 7700.
First / Last Name: Please type the first and last name as it appears on your insurance card if you are the primary card holder. Dependents will use their own name.

After creating your account you will then select a location from a drop down menu. You will then select the month and 15 minute time slot you desire. A confirmation email will be sent upon scheduling your screening.

Individuals must present a completed enrollment form at the screen. Please download this form and bring it with you the morning of your health screening.
